New Patient Registration

register_nowIf you live in our practice area and would like to register with us, our receptionists will be happy to help you with the relevant forms and will make you an appointment with a practice nurse for a basic health check. 

On the form you will be asked which practitioner you would prefer to see.  However, you will be registering with the practice rather than an individual GP. 

If you move outwith our practice area you should register with a new medical practice as soon as you move.

Medical treatment is available from the date of registration. Please contact reception for further information.

Alternatively you may print off a registration form,new patient questionnaire & consent for e mail & Text Messaging forms  fill them  out and bring it in with you on your first visit to the practice.

Consent E Mail & Text Messaging (16+)-Either print and complete the form or give permission verbally to Reception staff if you would like to sign up for this service, your consent will be recorded in your medical record. 

Medical Cards

These are no longer produced routinely but if patients require one they should contact Practitioner Services either through the website www.psd.scot.nhs.uk/contact-us.html by completing the online enquiry form or by telephone on 01224-358400

Change of Address

It is important to inform our receptionists if you change your name, address or telephone number so that your medical records can be kept up to date. Please fillout the form below and hand it into reception. Please note ID will be required for change of name.   Patients who move outwith our catchment area will be asked to register with a practice in their new area. Similarly patients who are already residing outwith the practice area, will be asked to register elsewhere if they move again and remain outwith the catchment area.
